目的：分析住院口服用药医嘱拒配的原因。方法：对华东医院2014-2015年住院药房不合理口服药物医嘱拒配记录进行归纳分析。结果：拒配问题主要是单次用药剂量错误（60.5%）和用药频次错误（25.2%），有63%医嘱如未拒配可能造成严重后果，拒配医嘱形成原因主要是跨科使用治疗药物。结论：药师要加强对跨科治疗用药的医嘱审核，特别关注单次剂量10片以下的用药差错，加强医药相互沟通，避免不良后果。

OBJECTIVE To analyze the cause of refusal records of oral drug prescriptions.METHODS The oral drug prescriptions refused from 2014 to 2015 in inpatient dispensary were collected.RESULTS The main reasons of prescriptions refusal were wrong dosage (60.5%) and frequency (25.2%). Unreasonable oral drug prescriptions which might cause serious consequences accounted for 63%. The main cause of unreasonable oral drug prescriptions was therapeutic drug from other department.CONCLUSION The pharmacists should enhance the prescription check for using therapeutic drug from other departments, especially the wrong single dosage less than 10 tablets.
